Differential diagnosis

- Cowden disease (similar histology, different clinical features)
- Crohn disease
- mucinous adenocarcinoma

  • irregular mucin pools,
  • epithelium floating in mucin,
  • complex glandular proliferation,
  • variable atypia,
  • desmoplasia,
  • usually no hemorrhage

- rectal endometriosis
- ulcerative rectitis / ulcerative proctitis
- ulcers due to ergotamine suppositories
