T-cell rich DLBCL

Definition: Diffuse large B cell lymphoma subtype with abundant non-neoplastic T cells and histiocytes.

- HPC:340 : nodal T-cell rich diffuse large B cell lymphoma (nodal T-cell rich DLBCL)

Synopsis

- Usually middle-aged adults, often with advanced stage and involvement of lymph nodes, spleen, bone marrow
- fever, pancytopenia
- Morphologic variants include L&H-like, centroblast / immunoblast-like and Reed-Sternberg-like
- L&H-like cases may be related to nodular lymphocyte predominant Hodgkin lymphoma

Differential diagnosis

- nodular lymphocyte predominant Hodgkin lymphoma (NLPHL)

  • has residual IgD positive mantle cells and follicular dendritic cell meshwork (not present in diffuse large B cell lymphoma)

Localization

- nodal T-cell rich diffuse large B cell lymphoma (nodal T-cell rich DLBCL)
