histiocytic infiltration

Friday 13 January 2017

histiocyte-rich lesion ; histiocytic infiltrates

Examples

- histiocytic infiltration in atypical mycobacteriosis

- lymphohistiocytic infiltration in digestive histoplasmosis

- histiocytic infiltration in Erdheim-Chester disease

Etiology (examples)

- infectious diseases

  • mycobacterioses
  • mycoses / fungal diseases
    • histoplamosis

- histiocytosis / histiocytoses

  • crystal storing histiocytosis

Localization

- histiocytic infiltrates of the skin / cutaneous histiocytic infiltrates

Diagnostic of histiocytic infiltration

- IHC : CD68 , CD1a , S100
- search for infections agents : Ziehl , PAS , Gram , Grocott , methenamine silver
- search for intracellulaire bodies :

  • Michaelis-Guttman bodies in malkoplakia

A CD68- , CD1a+ , S100+, langerin+ IHC profile makes a diagnosis of Langherans cell histiocytosis.

A CD68+ , S100+ or S100- , CD1a- , langerin IHC profile suggests a diagnosis of non-Langerhans cell histiocytosis as juvenile xanthogranuloma or Erdheim-Chester diseuse .

The presence of of Michaelis-Guttman Bodies m’aies thé diagnosis of a malakoplakia.

- infectious agents in histiocytes

  • The presence of an infections agent in the cytoplasm of histiocytes makes the diagnosis of an infections disease with histiocytic reation :
    • AARB with Ziehl : mycobacterosis as tuberculoses or lepra
    • deep fungal diseases as histoplasmosis
