Definition : atypical squamous cells with high-grade lesions that cannot be excluded (ASC-H).

The Bethesda System 2001: ASC-H: atypical squamous cells, cannot exclude high grade lesion. They represent less than 10% of ASC.

New term in ASC, to individualise cytologic changes of squamous cells that are suggestive of HSIL but lack criteria required for a definitive interpretation.

These cases are associated with a higher risk of intraepithelial lesions, more often with CIN2 and CIN3 than in ASC-US smears.

Types

- numerous naked nuclei in groups from reserve cell hyperplasia (RCH), with nuclear size variations
- isolated naked nuclei from reserve cell hyperplasia (RCH), with nuclear size variations
- isolated naked nuclei, hypo or hyperchromatic, from reserve cell hyperplasia (RCH), with nuclear size variations
- Large groups of immature atypical cells some with nucleoli (atypical immature metaplasia (arrow) or atypical repair?), sometimes more columnar-shaped
- cohesive cluster of atypical cells (immature metaplasia) with nuclear enlargement, hyperchromasia and anisokaryosis
- group of immature, atypical metaplastic cells with markedly enlarged nuclei
